Our assessment service provides a simple, accessible starting point for people looking to improve their health, wellbeing, and participation in everyday life. Led by Occupational Therapists, this service offers a high-impact assessment to understand each individual’s needs, goals, and the barriers they may face.

Understanding the Person
Through a holistic, person-centred approach, we explore daily life, motivation, strengths, and challenges to ensure physical activity is meaningful, relevant, and achievable.
Creating the Right Pathway
Following assessment, we co-create a clear and practical plan, including personalised goals, appropriate activities, and strategies to support confidence, independence, and participation.


Connecting to Opportunities
Our model takes a different approach to traditional signposting. Rather than placing the burden of navigating complex systems onto individuals, we take shared responsibility for that journey. We support people from the front door through to connection, providing active, short-term support to access the right opportunities in their community.
By walking alongside people — not simply directing them — we increase engagement and support long-term participation in meaningful activity.
