Our Primary Care service embeds Occupational Therapists within GP practices, working as part of Primary Care Networks to support people to stay well, independent, and connected to their communities. By working alongside GPs and wider teams, we provide proactive, preventative support closer to home helping people manage their health and daily life more effectively.

Supporting Everyday Life

Occupational Therapists in primary care focus on how health impacts a person’s ability to live their life.

We work with people experiencing frailty, long-term health conditions, mental health challenges, or complex needs – supporting them to:

Early Intervention
and Prevention

We provide early, targeted support to prevent decline and reduce the need for more intensive services.

Each Primary Care Network is different, so our service is tailored to local population needs and the specific challenges identified within the PCN – ensuring a targeted and responsive approach.

By intervening early, we help people stay independent and reduce avoidable GP appointments, hospital admissions, and crisis situations.
