A community-based prevention service
Reconnect is our place-based, early help and prevention service, supporting people to improve their health, wellbeing, and independence through meaningful activity. Developed through a Sport England-funded pilot, Reconnect is now a commissioned, countywide adult social care service in Essex – embedding Occupational Therapy and physical activity into how care is delivered.
Rooted in Everyday Life
Reconnect is led by Occupational Therapists and delivered in local community and leisure settings, working alongside coaches and partners to support people in environments that are familiar and accessible.
We work with adults with disabilities, long-term health conditions, autism, or sensory needs, as well as their carers focusing on what matters most to them and how they want to live their lives. Using a person-centred approach, we support individuals to build meaningful routines, increase activity, and reconnect with their communities.

A Different Approach to Care
Reconnect represents a shift away from reactive, crisis-driven care towards prevention. By embedding physical activity as a therapeutic tool, we support people to:
This approach is fully integrated within Public Health and Adult Social Care, creating a more joined-up system that supports people earlier and more effectively.
Delivered in the Community
Our programmes take place in leisure centres and community settings across Essex, where Occupational Therapists and coaches work together to create inclusive, welcoming environments.
Activities are shaped around the individual from swimming and multi-sports to strength and balance ensuring they are meaningful, adaptable, and aligned to each person’s goals.

Reconnect has evolved from a test-and-learn pilot into long-term system change, demonstrating how community-based, Occupational Therapy-led physical activity can transform health and social care.
By focusing on prevention, connection, and participation, Reconnect enables people to live well, stay connected, and take part in life on their own terms.
